Preparing for Your Initial Visit

In preparation for your first visit:

• Please arrive 15 minutes prior to your appointment to allow time to fill out the initial new patient information forms
• Please bring your insurance cards with you and be aware you will be responsible for co-pay at the time of your visit. We accept payments in the form of cash, credit, debit, or check.
• Your first visit at the Center will be an initial evaluation. Any additional testing required may need to occur on a separate visit.
• We will be doing an assessment of the pelvic area, so please dress appropriately with this in mind.
• Please feel free to bring a family member or a friend with you to your visit.
• Please contact us with any questions or concerns you may have, we are here to be a resource to you the patient.
